A word you learned wrong is harder to catch than a word you never learned, because a gap announces itself and a wrong meaning does not. The useful surprise is what happens after you catch it: errors you held confidently get corrected better than ones you held loosely, provided something tests you again afterwards.
The word you have been getting wrong for years
Almost everyone has one. Mine was nonplussed, which I used for about a decade to mean unbothered, because that is what it sounds like. It means the opposite: bewildered, at a loss. Nobody corrected me, because the sentences I put it in mostly still parsed.
That last part is the whole problem. A wrong meaning survives because it usually does not break anything loudly enough for someone to stop you.
There is a whole category of these, and they share a mechanism: the word resembles a different word, and the resemblance supplies a meaning before the dictionary gets a chance to.

Inflammable is the one with consequences beyond a raised eyebrow, and that is roughly why safety labelling largely abandoned it in favour of flammable. A word whose most intuitive reading is the exact reverse of its meaning is a design flaw, and the industry responded by removing the word rather than by teaching it.
The others are quieter. You can go a long time using noisome for a loud room, because a loud room is often also unpleasant and nobody stops to audit which kind of unpleasant you meant.
Why a wrong meaning is not the same as a gap
These two failures feel similar and behave completely differently.
A gap is self-announcing. You reach for a word, nothing is there, and you immediately know you do not know it. The feedback is instant and it arrives at exactly the moment you needed the word.
A wrong meaning produces no such signal. You reach, something arrives, and it feels identical to knowing. There is no internal alarm that distinguishes a confident correct retrieval from a confident incorrect one. That is why these can sit undisturbed for years while genuine gaps get filled within days.
There is a second-order problem too. Because the wrong version retrieves smoothly, it also gets used more often than a word you are unsure about. Uncertainty makes people hedge or reach for a safer synonym; confidence makes them commit. So the words you have wrong are, on average, the ones you deploy most freely, which is an unfortunate arrangement.
So the difficulty is not really in the fixing. It is in the noticing, and the noticing has to come from outside you: a person who queries it, a dictionary you happened to open, or a sentence somewhere that your version cannot account for.
Confident errors are corrected best
Here is the finding that runs against intuition.
You would expect a firmly held wrong belief to be the hardest to shift, and a vague one to be easy. Research on error correction has found the reverse for factual knowledge: errors committed with high confidence are corrected more reliably than errors committed with low confidence, once clear feedback arrives. The finding is called the hypercorrection effect, from work by Butterfield and Metcalfe, and it has been replicated across different materials and age groups, including children.
For vocabulary this is genuinely good news. The words you are most sure about, the ones you would defend, are the ones that will actually move once someone shows you the entry.
Why surprise does the work
The explanation is about attention rather than about memory strength.
When you are confident and the feedback contradicts you, there is a gap between what you expected and what you got. That mismatch is surprising, surprise pulls attention, and you process the correction more deeply than you would have processed a fact you were only half sure about. A low-confidence error produces a shrug. A high-confidence error produces a small jolt, and the jolt is doing the encoding.
I find this reassuring in a way that most memory research is not. The embarrassment of having been wrong out loud is not just a social cost. It is most of the mechanism.
The condition almost everyone skips
Now the part that gets cut from summaries of this effect, and it is the part that decides whether any of it helps you.
The hypercorrection advantage holds at a delay when a test comes after the correction. When no test intervenes, high-confidence errors have been shown to return. The correction does not permanently overwrite the old version; it competes with it, and which one wins later depends on whether you have retrieved the correct one since.
This matters because of what people actually do. You discover you have been wrong, you feel the jolt, you read the correct definition, and you close the tab. That sequence feels like resolution and contains no retrieval at all. You have read the answer, not produced it.
It is worth being precise about what the studies show, because the two halves point in different directions and both are useful. The hypercorrection advantage survives a week-long delay, so the effect is not a laboratory artefact that evaporates by Tuesday. But high-confidence errors have been observed returning at a delay when nothing tested the learner in between, and not returning when a test did. Correction plus a test holds. Correction alone is the version that decays.
The fix is not more reading. It is one deliberate retrieval a few days later. That is the whole argument for active recall, applied to a single stubborn word.
How to find the ones you have wrong
Since these words do not raise their hand, you have to go looking. A few reliable tells:

The last two are worth acting on. If you cannot recall reading someone else use a word the way you use it, that is not proof of anything, but it is the cheapest check available and it takes one search.
What to do when you catch one
The sequence that actually holds:
- Say what you thought it meant, explicitly. Out loud or written down. Naming the wrong version is what creates the mismatch that the effect depends on
- Read the real entry, including the example sentences. The examples carry the register that the definition line usually omits
- Write one sentence of your own using it correctly. Production, not recognition
- Test yourself again in a few days. This is the non-optional step. Without it the old version can come back
- Expect one relapse. Catching yourself about to use the old meaning is the correction working, not failing
Step 4 is the one that gets skipped, and skipping it is what turns a real correction into a temporary one. It does not need to be elaborate. Covering the entry and saying the meaning out loud counts; so does using the word once in something you were writing anyway. What it cannot be is rereading the definition, which is the thing your brain will offer to do instead because it is easier and feels equivalent.
Step 5 is worth expecting rather than fearing. The old version does not vanish the moment you learn the new one, so for a while both are available and the wrong one is better practised. Catching yourself mid-sentence is evidence that the correction is installed and competing, which is exactly the stage before it wins.
Where this does not apply
Two honest limits.
The research here is largely on factual knowledge and general-information questions rather than on second-language vocabulary specifically, so I am extending it by analogy. The mechanism, surprise driving attention to feedback, has no obvious reason to stop at language, but I would rather flag the extension than pretend the studies were about vocabulary decks.
The other limit is bigger, and it is why the table above stopped where it did. Some words are not wrong so much as contested, and several of the most-cited examples in this genre belong in that column rather than the error column.
- nonplussed: bewildered, but a newer sense of unfazed is widespread enough that dictionaries record it
- peruse: traditionally to read closely, now very commonly to skim
- enormity: traditionally great wickedness, now routinely great size
- fulsome: traditionally excessive and insincere, now often simply abundant
- disinterested: impartial, but constantly used for uninterested
Each of these has an older sense that a certain kind of reader will defend and a newer sense that most people actually use. If you use one of them the modern way you are not making an error, you are picking a side, and the honest move is to know that a side is being picked. Correcting yourself into a rule that nobody follows is its own kind of mistake, and it produces writing that is technically defensible and slightly odd.
The practical filter: if the two candidate meanings are unrelated, you probably learned it wrong. If they are two stages of the same drift, you have wandered into a usage argument.
What your app can and cannot do here
Software cannot tell you that you learned a word wrong before you meet it again, because it has no access to the version in your head until you produce one.
